Opt-in skill that appends two or three short, targeted follow-up questions after a substantive answer the user may act on, helping them check key facts, probe the reasoning and assumptions, and notice missing context before relying on the answer. Fires at most once per conversation, with explicit skip rules for trivial lookups, formatting tasks, code, and creative writing.
